If you look in the above shot, the circle on the right shows the trail left by the first stage. The altitude at the end of this part of the launch would already have been much higher than what we call “atmosphere”. (perhaps already as much as 100km)
The blue trail is the next stage, and is yet higher than the first stage. The spiral was the last stage, where the problem with the missile occurred.
